Is it safe to assume that there is already a register of all legal immigrants and refugees entering the U.S.A. If not, why not?

Of course legal immigrants register when they enter the country for the first time. This is how they get a Green card.

Is it safe to assume that there is already a register of all legal immigrants and refugees entering the U.S.A. If not, why not? Because if the government had set out to establish it ten years ago it would be billions of dollars later and they still wouldn't have it working right.
