It looked like they knew there was a maintenance issue, and two female employees were at the top as the woman and her child came up. The child's father was the smart one, staying at the bottom. It appears the woman stepped on the escalator despite a warning notice that it was being worked on.

Looks like negligence by the store, they should not have allowed anyone on it until fixed and tested. The top access plate was not properly secured. If true, it should have been roped off until secured.

In any case, a horrific video to watch as the poor woman was pulled into the machinery.

I seem to recall something similar happening a few years ago in China with an escalator.
