Mostly yes. If not mentally ill, then emotionally damaged and emotionally ill from trauma experience earlier in life. You are dead on accurate with the observation of self-loathing. It is, sadly, a core outcome in one who has been victimized and emotionally beaten in childhood.
Homosexuality or false gender identity is occasionally a manifestation of this trauma, but only occasionally. The self-loathing is, perversely, a core and frequent feature, with other frequent manifestations of depression and anxiety. Suicides and other negative actions are the consequence of the original traumas. Those who caused these traumas should be the target of inquiry, not the therapist who desires a healthier patient.
