In this article (http://freebeacon.com/iowa-pastor-diagnosed-with-cancer-finds-obamacare-anything-but-affordable/), it says his ACA insurance doesn't start for another two weeks, and that he finds the price of the ACA insurance that covers the treatment to be unaffordable. So it's his old insurance that doesn't cover the treatment. And basically, he's ticked that Obummercare doesn't pay for the costs racked up by his old insurance. My sympathy more or less goes away at that point.

Most of these cases stem from someone having coverage, then losing the coverage they were happy with and were paying for and which covered more. Then getting bounced from "you can keep your doctor if you want" lie Obama told us. Thus having to apply for Obamacare "cold stick" with some exchange you don't know, and they don't know you like your old insurance company, then getting sticker shock that the plan Obamacare presents you is more expensive and covers less. That seems to be the issue with these cases.
Then again, the drones in the Beltway could have just passed a healthcare law which covers those who did not have insurance, one in which protects those with pre-existing conditions and just overwrite it under one program and leave everyone else alone. No but they had to take total control of our lives by passing the monster they created.
