When this came up on O’Reilly tonight, I told the TV set that “O crap, they left a loophole, that’ll have to get fixed”. I doubt they even considered that doctors might not want to be a part of Obamacare, but now that they know it some changes will be needed. Probably via the IRS, as even rebellious doctors might respond to a highly punitive tax on non-Obamacare income.

On the other hand, there are probably going to be some very nice sunny islands that have a very high doctor-patient ratio.

first they will entice doctors to sign up by paying their malpractice insurance or medical school debts for them. Then they are trapped.

doctors might not want to be a part of Obamacare,

In their own repugnant arrogance they know they can “create new rules and regs” in ObamaramaCare with Reid dealing the cards and Boehner asking how high.

I think I’d look at the German system. There are doctors in Germany who will not take the state-run public insurance card. So you make an arrangement involving cash. The doctors who typically fall into this area....are your better-than-average doctors, who know they are better than what you get with the public clinic concept. The positive with the cash arrangement? You tend to get seen...even on short-notice.

Or how about off-shore hospital-cruise-ships, sort of like the casinos use?

One country in Europe (Luxembourg?) actually drafted doctors into the army when they rebelled. That’s another option that might happen here.
