Well, based on evidence so far, including physical, debris trails, impact marks, witness statements and video, it appears this pilot and those in the cockpit, totally screwed up...
Too slow, too low, and way too short.

Earlier in the approach they were way too high (over 600 ft over where they should have been).
The flightaware data shows this was a very bad approach from the start.
