I agree. It's also government collection of household information under threat of penalty. No way I'd have anything to do with this form!

From page 1: We'll keep all the information you provide private, as required by law. I saw nothing on any of these pages which limited exchange of information among government agencies or gave any definition of "as required by law."

From page 6: Check out the OTHER INCOME you're re require to give for your spouse. This is something most people don't know about yet. The IRS will be in regular contact with employers about household income, not just the employee's insurance through the employer. You can see more evidence of this on page 15.

From page 18: I know that there may be penalty if I'm not truthful. What kind of penalty? I know that my information on this form will only be used .... No, I don't know that! They haven't specified exactly what, if any, privacy guidelines there are, yet the applicant is asked to sign "I know...". What BS! Again the phrase will be kept private as required by law is used. Then it states, I know that I must tell the Health Insurance Marketplace if anything changes (and is different than) what I wrote on this application. So we can see that the Marketplace is an information collection and benefit assignment tool, and NOT really a marketplace at all. The applicant isn't buying anything. He's being assigned a price for a product he's not free to choose or he's routed into a government program.

More from page 18: I know that under federal law, discrimination isn't permitted on the basis of race, color, national origin... No, I don't know this in these days and times. In fact I wonder about racial preferences given the number of questions related to minorities, especially hispanics.

More from page 18: The applicant is asked to confirm that no one on the application is incarcerated or in a medical facility. It's not specified at what point the applicant must inform the marketplace if anyone on the application falls under these categories or what constitutes a medical facility.

More from page 18: Now they're back to family income again. ...the Health Insurance Marketplace will use income data including information from tax returns of household members. This will determine yearly eligibility for help paying for health insurance for the next 5 years.

From page 19: The authorized representative in effect becomes a POA. There doesn't seem to be a lesser option to authorize someone to assist you (i.e. get information, ask clarifying questions) without at the same time authorizing them to take action on your behalf.

More from page 19: Did you remember to: Tell us about everyone in your family & household, even if they don't need insurance? Now it's not just household, it's family AND household.

Man, I want NOTHING to do with this nonsense!
