ObamaCare is a “law” for which one can be prosecuted.
Remember, the invocation of 5A protection tosses the onus of evidence back on the prosecution (they have to prove you are not 5A-protected).
Most of it is repetition, depending on how many member there are in your family. Looks like there is a single 2-page section you have to fill out, and then there are five more 2-page sections for other members of your family. If you are just a single earner there is about four pages. One page of basic contact information, two pages of income information, and one page of current insurance information.
Problem is, there is no such website. Try it and see. I assume they won't activate the website until just before they officially distribute the forms, but I HAD A DAMN QUESTION.
Depends on what you mean by the word 'coverage'.
You can sign up, and 'buy' the new 'government' insurance, but the only thing that I think they will be denying anyone is 'health care coverage'.
The departments managing the Obamacare mess will blow past that.

That's been tried with not filing tax returns and it doesn't work. The courts essentially said "yes, you have a right to 5th Amendment protection, but that doesn't relieve you of the responsibility to file a tax return. A tax form with "5th Amendment" scrawled across its face does not constitute a "return""
