James Holmes came to the attention of the threat assessment committee at the University of Colorado but no further action was taken because he left the school more than a month before the attack that killed 12 and injured 58, sources told ABC News.
ABC News has learned that Dr. Lynne Fenton, the psychiatrist who was treating Holmes, 24, at the school, was also a key member of the university's threat assessment team. The group of experts were responsible for protecting the school from potentially violent students.
KMGH-TV, ABC News' affiliate in Denver, reported exclusively that, according to sources, by early June, Fenton had informed other members of the team about her concerns regarding Holmes.
But on June 10 -- three days after Holmes bought an assault weapon and added it to his already growing arsenal -- he suddenly told the university that he was dropping out of the neurosciences doctoral program with no explanation.
KMGH-TV reported last week that he'd purchased the weapon hours after failing a key oral exam.

If this exact picture was in your newspaper next to a story about her running a marathon for charity, or if she walked by you on the street, you wouldn't even notice or have the slightest thought she was "evil."
Same thing goes for the first pic of Holmes himself (not the one with the dyed hair); just a slightly-goofy looking picture of a 24 year old guy that doesn't photograph well. Yet people were proclaiming how they could see the evil in his eyes, or claiming to derive his entire set of political views from the picuture - you're just seeing what you want to see.

Sources have told KMGH-TV that the threat assessment team never had a formal meeting and never intervened
Michael Carrigan, chairman of the CU board of regents, told KMGH that he did not know if Holmes had ever been discussed by the threat assessment team. "It's the first I'm hearing about this," he said in a phone interview.
A CU spokeswoman declined comment to KMGH on Fenton or any threat-team team actions, citing a gag order.
Don Elliman, the university's chancellor, said last week that "to the best of our knowledge, at this point, we did everything we think we could have done."

She was this killer’s psychiatrist. She was very concerned about him. Then how come she let his package sit in the mail room for 7-10 days? How come she did not accept delivery of it and open it up? The package was sent before the killings. She might be very culpable for not opening it up. Some accounts say his notebook was in there

Indeed. There isn’t much anyone could have done that we’d want to live with. Extreme outliers are hard to detect, and hard to act on: our social system doesn’t know what to do with them (the “team” was academic, not police), and is prone to normalizing views of outliers in hopes they’ll go away; to implement a system which would catch & stop his kind would require intrusive police powers other FR conspiracy theorists regularly shriek is coming.
