Often a young, extremely pregnant (or not so pregnant) woman doesn't DECIDE to give birth. It just happens, and she can't stop it. The birth happens wherever she happens to be at the moment. Also, medical care is more prevalent in the larger cities.
Which is why very few women, even today, decide to travel all over the world in their 9th month of pregnancy.
