We had one in the town that I grew up in, and in remembering back, it was a combination of both the older crowd, and the absolute lack of brain powered people working either the register, the ice cream window, or taking your order
I would bet that there was a map behind the counter to show the workers how to tie their shoes if their laces ever came undone.
But the burgers were good.. : )

I never thought it was so much lack of brainpower as ambivalence/apathy.
There was one night I was at my local one with friends and we finally got fed up with how long it was taking to pay. So we left our tip by pooling all of our loose change, dropping it into a completely full glass of water, covering the top of the glass with a menu and flipping it over onto the table ... then sliding the menu out.
We made sure not to go back there for a while after doing that ...
