Good find!!!! What IS that about .. white AND green forms?

The letter “K” in box #16 doesn’t match the rest of them. Why no mailing address for the mother?

Look closely, and you’ll see the pattern from the green background version in the shadows of the page curl on the left. This black and white one is obviously a photocopy of the green one. The green area is probably non-repro (doesn’t photocopy).

It's like they took the white one and superimposed it upon the green paper. Most all of the writing whether typed or written has a white cloud around and behind it on the green copy, that is except the large caps of signatures which has painted green blobs in them the shape of the empty spaces in the letters rather than the green lines of the actual paper which should have been evident in the large empty spaces of the caps. Maybe they realized after they made the white copy that it was a mistake because it should have been on green paper and then tried to very carefully superimpose the white original copy letters and numbers on the green paper leaving the white clouds around much of the lettering.
