I just ordered my (what they call) Birth Certificate from Hawaii, on-line. The total cost of ordering 1 BC was $11.50 using the online form.

I was born in Hawaii in 1968 for anyone’s curiosity.

The online ordering system was very basic. It never gave me the opportunity to say if I wanted the short form or the “Certificate of Live Birth” which I have. (long form)

I ordered it, received a receipt, saying it would be sent out in about 2 weeks.

So I followed up with a phone call to tech support to ask my question about which form I would receive. They told me they couldn’t answer my question, but gave me a couple of numbers to try. These numbers are constantly busy. I will keep trying.

I then sent an email to “vr-info@doh.hawaii.gov“. This was the email given to me over the phone.
I asked in the email which form I would be receiving after ordering my BC online.

I will let everyone know if and when a response is received. Or when I can get through on the phone.

Of course I will let everyone know what certificate Hawaii sends me in the mail.
