Thanks for the correction. Hawaii DOH says they HAVE a ‘vault copy’ (photocopy) of the original “long form”; I did not know that.

But who gets to see it? If you request any sort of ‘proof of birth’ from Hawaii DOH, what they send you is the computer printout form.

Is there a mechanism whereby an individual gets to see their original long form? Because the infamous “$12 and this all goes away!” lament is deceptive in that $12 to the Hawaii DOH buys you a computer printout form, NOT the much ballyhooed “long form”.
