I’m reminded of a short story by Philip Jose Farmer called “Caught in the Organ Draft”. In the future, to keep senior citizens alive, young people are “drafted” for organ harvest.

If it were legal to buy and sell organs, the organ shortage would go away overnight. People would have more of an incentive to donate their organs when they die to help their family financially.

Anybody remember the mover “Coma”. Mandatory organ donation is a scary thought if you have seen this movie AND the Federal government is successful in taking over the healthcare industry.

How Libertarian of you. But also how true. Of course there would be problems with setting up such a system such as "What if someone decides to opt out after they've received the payment" or (if payments were only made after harvesting) "What if the donor had no next of kin to receive the payment?"
All of these, however, are much easier questions to answer than setting up a government bureaucracy to decided who deserves the organs or who deserves to donate.
You could start it out very simply-- if you are a donor, your estate will get x$ toward your funeral or burial expenses if something is harvested. Even a capital criminal who cares about nobody might be persuaded to donate with the promise of a nice tombstone versus a brass marker.
