Is there really such a prayer saying “partners?”

I am seeing things about God and a Book of Life, but not “partners” as in decision making.

No. The "who shall live and who shall die" is in the Rosh Hashanah Liturgy. There is a passage in the Talmud, Kiddushin 30b which says, approximately "there are three partners in making a person, the Holy One Blessed be He, the father and the mother. That is an entirely unrelated subject. We are not given dominion over our own bodies, let alone those of others. See Rabbi Mordechai Biser's THE NEW IDEOLOGY IN HEALTH CARE AND HOW TO SURVIVE IT Jewish World Review 2004/5765 for some discussion of this. That this conference should occur on the eve of Rosh Chodesh Elul, 30 days before Rosh Hashanah is profoundly dismaying to me, as is the fact that it would appear that none of the participants in this conference have presented the authentic Torah view.
