Are we misreading it? Could the zero in front of BOs actually be a the letter O. (For Original)? Then altered once be listed as A10641 or B10641 as court ordered changes took place. Do we know ANYONE In HI that know how the notation is arrived at or changed for legal purposes?
They widened the number from five to six digits sometime between 1962 and 1970. All the short-form certificates have six-digit numbers. When and if we get to see Zero's long form, its file number will have a two-digit year and a five-digit sequence number, just as do the Nordykes' long forms.

In 1960, 632,772 people lived on the island. By 1990, the population nearly doubled on the islands to 1,108,229. The Certification numbering system adds that extra zero "0" in front of the last set of numbers to ensure a consistent numbering system through the 21st century.
The numbering system has evolved as the population has grown.
For example, in the 1920s when Hawaii was a territory, they did NOT see the need to add the year group in front of the birth certificate #, as this 1927 "Standard Certificate of Birth" shows. In 1920, there was only 255,881 people on the island. The population grew nearly 45 percent to 368,300 by 1930!
