"OHSM 1.1 (Rev. 11/01) LASER"

From my experience, I'd say that government forms generally carry a revision and date number. I do not know if "11/01" refers to "November 2001" and I wonder, if that is so, why it would be appearing on a document issued in March 2000. It seems far more likely to me that it would read "11/99" (that is, November 1999) instead.

I do not know. I remember some discussion of document revision dates, I think it might be on the Obamafile, I will go look there as well.
