The "Certificate of Live Birth" is the "long form" birth certificate. It would the mother's signature, the doctor's signature, a hospital "registrar's" signature or for a birth outside of Hawaii, supporting information. There would also be supporting information for an amended certificate or a late filing (more than a year after birth). The most common reason for such an amendment is an adoption, where the adoptive parents' names replace the birth parents' names. But a legal name change will also be applied to the BC. The certification will only show the most recent data.
