Ohio requires either a photo ID even if voting absentee in person at the election board or the the last 4 digits of your SS#, or your drivers license # or a copy of utilities bill that show the current name & address and it must match what is on the books in order for your vote to count

Is that other information submitted with the ballot?

Yes, that information is on the envelope that the absentee ballot is in. It is verified before the envelope is even opened. Even if voting provisional you must submit the same information. I cant say that all countys follow this procedure but where I live they do. Watch on election night for Cuyahoga County to have problems, they always do when no other have problems.
