It may be an old saying, but in this particular context it was obviously a reference to Palin, and the audience clearly took it that way. If he wasn't referring to Palin, who or what was he referring to? But you have a point - obviously his statement could reasonably be interpreted as him calling Palin a pig, so it was foolish for him to say it and think that he wouldn't be called to account for it.

When listened to in its entire context, Obama wasn’t referring to Palin specifically. He was referring to the same ol’ policies, 8 years of Bush, bla-bla-bla, not being anything about change...HOWEVER, to throw in the lipstick on a pig comment was a calculated move on his part, knowing full darn well the audience would immediately associate it with Palin’s previous lipstick comment. Even if it was an impromptu comment on his part, it was so totally tacky, totally unbecoming of someone wanting to be the POTUS, leader of the free world. And frankly, the fact that McCain said it earlier in the year with regard to Hillary’s health care plan, was tacky and unbecoming as well.
