Now i think the difference is: one means the application/info was submitted by a state recognized entity, i.e. legitimate hospital, and the other one means the info was just ‘taken and filed’ but not accepted as true. So that brings up the question of why?

He needs to show us one like this... I was born in 66 and mine looks almost exactly like this. I wasn’t born in IL either. So I know he has one like this.
