The street rods sites are saying it was an inherited (meaning not alcohol related) liver failure combined with diabetes.
More than likely aggravated by long-term exposure to paints and solvents-which is very hard on the liver.

Several people on a hot rod website I visit know Duane and they say he is not like that in real life.That's often true. I quit watching the show because I only liked two of the guys. Charlie, the painter, who seemed far more capable of running the shop than Boyd or the foreman who seems to have chronic PMS. The other guy I liked was the grouchy old fabricater whose name I forget but had the attitude of "just leave me alone and I'll get it done." (He was the one that worked with the black guy who talked all the time.)

Don’t mix up contrived TV drama with the reality of who the man was. He was a very generous person, ran a Charitable organization and was an icon in his field.
May he rest in peace.
