Just for yucks magnify the picture and compare the heads. You’ll see the weak blending attempts along the edge of Obama’s face versus the old man’s. Lighting’s different too. I am fairly sure it’s a fake.

His hands are correct, the shirt is as on other photos in kenya, nothing odd with the light. The face does not have odd blur lines. The very same blurs you mentioned are patterned all over the image. Right next to the section you outlined you can find that blurs again. The entire image is not 100% sharp. This photo is real. The "blurs" caused by the grainy nature of the print(!!!) version (printed newspapers images are usually grainy), which was scanned and enlarged.
