Yes. Accidental center fuel tank explosions have occured in two Boeing 737s.

No.
Catastrophic airliner mechanical failure modes are, obviously, rare, or planes would be falling out of the sky left and right.
Only one engine has fallen off a DC-10 in flight, for example.

Much older versions of the fight 800 747 too. The 747 had already had the retro to prevent such a problem. Most of the pump motors are encased in a non conductive epoxy resin type material. It won't a fuel pump short that caused the explosion, IMHO.
Ever wonder why the Feds sealed the evidence, and destroyed some, from the public after the 'official' explanation? IMHO, if it were true about a fuel pump spark, there should be no reason to seal and destroy the evidence.
