Surgery probably means it's more serious than a "normal" stroke.

Coworker's sister came out of a stroke that left her not speaking. Between the drugs and the surgery, they brought her back with "minimal" damage (some problems with short term memory.) He should have the best of care there. But whoever let him sit in the office for that long after the first episode was no friend to him if this was a stroke or aneurism.

My early guess was a Aneurysm and they would do surgery for that. My sister survived one in 1981 after a couple of operations and 3 months in intensive care as she was completely out of her mind. They finally guessed her shunt from her brain to her stomach had plugged up and they installed a new one.

She came out of the comma in a few days but my wife had to totally reprogram her to walk and talk. Six months of hell for my bride but sis recovered enough to drive again even with the her right leg drop. She is still alive today although she in now housebound at 86.

The year she had her event there were 6 others who died without leaving the hospital. Another possibility is a brain tumor that would involve surgery.
