It's sad, but we insured patients have to pay $500 for the ambulance ride,$750 here in Columbus.

Our local community has an "ambulance association" or something like that. Costs about $100 per year to join per family. Then if you go to the ER by ambulance, whether you have insurance or not (we do), it is no charge. All the local hospitals, ambulance providers, and fire departments are in the pool so we are always covered.
Didn't need it for 12 years then in one year my wife needed it 3 times due to severe reactions to medications. All trips were covered.
Everyone should check with their local municipality or county and see if they have or know of one. Well worth it.
