Ignorance of the threat. Total focus on the (very highly visible) "airplane" above the real (but invisible) threat from below.

navy still has that problem: Local community focus. An officer is likely to spend 15 or 18 YEARS in one community before he even gets onboard another tactical part of the problem, much less go on cruise with it to understand what other side (sub or air or surface or amphib or refuel) can do, and CAN'T do.
