On the substantive issue of the adequacy of the Bush Administration's management of DoD, I have one salient comment:
Each individual case of friendly fire casualties is properly investigated on its own merits.But in the case where your side has a great preponderance of the firepower but a finite ability to target bad guys and not good guys, half of your casualties should come from friendly fire! If not, then management is not getting the right amount of aggressiveness out of the troops.
If you are not losing any people to friendly fire, you are allowing the enemy to kill more of your people than you'd lose if you ratcheted up the aggressiveness of your air and artillery. If more than half your casualties are by friendly fire, you need to have air/artillery stand down somewhat.
That is only a statistical statement, not a commentary on the preventablity of any given incident.
