Could also mean hyperthyroid with the white of the eye (sclera) visible all around. Might explain her thin face. And her behavior (manic episode?).

Had a great aunt that was hyperthyroid, painfully thin, too hyper. Then she got crazy as a loon for awhile until they removed part of her thyroid. This could explain the whole thing.
