That's just it...you don't have to go through with any operation as the term transgender means:

In females, the sex chromosomes are the 2 X chromosomes. Males have one X chromosome and one Y chromosome. The presence of the Y chromosome is decisive for unleashing the developmental program that leads to a baby boy.

It's not that I thought the guy's arms were very muscular, just distinctly not feminine. The wrist bones are just too thick. Even women who weight train don't lose the natural dainty shape of the female arm (unless they really over do it), but there's nothing this guy can do to change his male skeletal structure.
