Creating a mass infection/poisoning is more difficult than most people imagine--most cities of any size have alternate hospitals to call on for help in catatastrophes--but that last one on your list is chilling. Peds ward. ...!

Hospitals are the very place in a small community that is already being trained (via mock-catastrophe drills) to deal with terrorism. There are even conferences specializing in scenarios available as "continuing medical education"--CME.
