Yeah ... "recommends." That's the key word.

I'd agree that even this goes beyond the government's
legitimate business. But the government cannot require
everyone to be screened. Are there people who would
like this? No doubt. Will it happen? Look, I'm no
optimist about most things political. But your
nightmare scenarios are far-fetched this time. There
are many worse threats to worry about.

Look, I'm no optimist about most things political.

Actually I’m an ardent pessimist about most things political. I realize it would be difficult to force everyone to be screened - but not impossible. There was a time when I thought it would be impossible for many of the actions included in the “Patriot” Act to be to implement, but it happened.
All it would take would be to set up the proper scenario. Tying it in with universal health care is a start and brings the democrats into the fold. Make a case for “National Security” of the “War” on crime or drugs or whatever and it would be even closer to reality. It could be couched in terms that would make it appear to be “for the public good”.
I do sincerely hope it never happens, but I put nothing past politicians. Most of them are out for personal power and to make their mark on America - and could not care less for such ethereal things as “freedom” or less government interference in private lives. Some even believe that such draconian measures would make the USA a better place - a homogenous population, with every free person thinking the same way.
