The Challenger did not blow up due to takeoff damage. It was due to o-rings or something like that. You're thinking of the recent one - that I cant think if right this minute.
There are some 200 across the US. A few threads back, someone posted a map and pictures. I think information can be found on the Federation of American Scientists site.

June 11, 2004 A semi truck fire closed down part of I-80/94 near Gary, Ind. One of the truck's tandem trailers is fully engulfed in flames. The area near 27th and Van Buren in Gary has been evacuated.
Officials stopped traffic in both lanes of the freeway near mile marker 9.2 at first but have now reopened the right two lanes on the eastbound side. The westbound lanes remain closed.
No injuries are reported.
Police do not know what is in the trailer.

Fuel leaks- Bethesda Maryland
by Chris Williams
Staff Writer
June 11, 2004
A small amount of diesel fuel may have entered a Woodmont Avenue storm drain during two separate fuel leaks in Bethesda this morning.
At about 8:10 a.m., Montgomery County Fire and Rescue workers arrived at the 7600 block of Woodmont Avenue near Old Georgetown Road and Edgemoor Lane for a reported fuel spill, fire and rescue spokesman Pete Piringer said.
Hazardous material workers and firefighters quickly contained a small leak from a Ride-On bus, but labored until shortly after 10 a.m. to control the gushing 200-gallon fuel tank of a moving van, Piringer said.
Firefighters are unsure what caused the leaks, Piringer said.

Incendiary device found near Seattle ferry:

Potential incendiary device removed from near state ferry terminal
The Associated Press
SOUTHWORTH, Kitsap Peninsula
An apparatus resembling an incendiary device was removed from a small boat near the state ferry terminal in this Kitsap Peninsula town early this after six ferry runs were canceled, authorities said.
The device was a motorcycle-type battery about half the size of a car battery with wires leading into a brown bottle about eight ounces in size, Washington State Patrol Lt. Helmut Steele said. The bottle was less than half full of an unknown liquid, he added.
A bomb squad "rendered the device safe" and it was removed for further investigation, including identification of the liquid, Steele said. He would not say how the apparatus was neutralized or taken from the scene.
Ferry crews were cleared shortly before 3 a.m. PDT to resume service between Southworth, east of Port Orchard, and Vashon Island, west of Seattle in Puget Sound, ferry system spokeswoman Susan Huether-Harris said.
Three runs in each direction were canceled after a passenger getting off the ferry noticed a 12-foot aluminum skiff tied to the dock at the terminal about 9:45 p.m.
Thursday and alerted a crew member who took a closer look and spotted the device, Steele said.
A crew member untied the small boat from the dock, and it drifted onto a nearby beach and was tied to a piece of driftwood by the time the first state troopers arrived, he said.
Ownership of the skiff was under investigation.
No written threats or warning notes were found, nor did investigators know of any earlier verbal threats, security problems or other sign of trouble at the ferry terminal, Steele said.
Southworth-Vashon ferry service is part of a run linking West Seattle, Vashon and Southworth. Service between Vashon and the Fauntleroy dock in West Seattle was not affected.
