Sex has nothing to do with sexuality OK, thats interesting but I am not sure how it answers my question. Unless you are saying that ones gender has nothing to do with ones sexual preference, which I already know. I guess I didn't ask a very good question. I am just trying to understand the motivation for butchering ones genitals.

I don't think you are asking the right question. There is considerable variablity built into the human species as a survival trait. A consequence of that variability, is that some combinations are possible that will cause anomalous development. Consequently, some children are born intersexed. Intersexuality also extends to the way the brain develops in the womb.
A simple way to look at it is, consider the human genome as a deck of cards. The shuffling of the cards will produce individuals who are better suited to meeting environmental challenges on demand. However, sometimes an individual will get a lousy hand, such as being intersexed or being a transsexual.

Environmental challenges? What is that? Too far to walk to the sporting goods store, Crate & Barrel and Pottery Barn are closer, so you surgically change your genitalia and start taking hormones?
Transgender people are sick, PERIOD.

No! Those individuals have ambiguous genitals. They will identify as one gender or the other. They will need surgery to make their genital structure consistent with their gender identification.
A good example is the case of individuals with partial androgen insensitivity syndrome. They may have genital structure ranging from almost totally female to that to almost completely male in appearance.
They may well identify as women, however because of their ambiguous genital structure, they will have trouble functioning as women in society. On the other hand, they could identify as male. The growing medical consensus is to allow these individuals decide on the gender to which they belong. Doing so is more conducive to their well being.
