I have seen pages missing, notes long lost suddenly "reappearing", orders never entered in charts, signatures forged....you name it.

And I have been in 1000's of charts in my job....Many are total train wrecks. Some facilities so poorly run you are amazed that they are still open.

Her story sounds like stuff I have seen in other facilities.

Just depends on where you are and who you know.

I increasingly suspect that what's happening here is the nationwide practice of an industry is being revealed, and that big bucks are at stake. Like with the abortion industry.
