The US ensign when thus flown upside down is a distress signal.

The writer obviously doesn't understand the difference between an "ensign" and a "flag."

The two words are not synonymous!

To be fair, an "ensign" is not always visually easily distinguishable from a "flag." It has more to do with the situation in which it is hoisted (e.g., onboard a ship, just prior to entering battle).

Though some ensigns do differ markedly in design from the standard flag.

Here, it's probably due to mere sloppiness on the part of the writer.
