I believe its some sort of Shiite Muslim blood-letting festival in which men cut their heads and parade though the streets in commemeration of some long-ago "martyrdom". It may also involve whipping oneself bloody.
For the very youngest males, relatives, such as this doting mother, have to assist in the head slashing. I think the photo was taken in Lebanon.
