The avascular necrosis could be due to the inflammatory response to infection in the disease. That's what seems to cause the lung damage.

What do you expect from Reuters? Their bias is legendary.
However, steriod use is a known cause of AVN. The cause of the vast majority of cases of AVN is never known but if there is a history of steriod use, then often that can be tied to the AVN. My husband has AVN in both hips. He never used steroids as far as I know. So, unfortunately, he is in the the group where we never know the cause. I can tell you it is very painful.
