The elevator had a small window in each of the doors on all floors. There was a button to ring to call the elevator. The button wasn't working on that floor and as a consequence, someone had knocked out the window so they could hear if the elevator was moving. The guy stuck his head in the window to see where the elevator was. Unfortunately for him, it was on the floor above him..moving down. You can imagine the rest of the story..

Yuck, another freaky decapitation by an elevator! Who would have thunk it?

Something like this happened about 10 years ago in an apartment building in NYC. A woman got her heel caught in the space between the floor and the elevator, and a man bent over to help her free it. She got her foot out, and for some unexplained reason the elevator car went up without the doors closing first. The people in the elevator rode up 5 floors with the guys head in the car.
