CAPE CANAVERAL, Fla. - A Nobel Prize-winning member of the board investigating the space shuttle Columbia disaster says he fears NASA may be doomed to suffer more tragedies unless it changes the culture that has led to flawed decision-making.
The "same faulty reasoning" that led to the 1986 Challenger accident also led to Columbia, said Douglas Osheroff, one of the 13 board members wrapping up the report on the Columbia accident.

NASA's troubles sound like the predictable result of the educational culture in 80% of the colleges from which 'talent' is drawn:
Cheating overlooked, encouraged, twistedly respected;
Never accept blame or responsibility;
Low expectations, and half-efforts to meet them are acceptable.....and rewarded. And I reckon this all flows from the primary and secondary "self esteem" culture. The kids and parents, who pay the freight, are demanding the paradigm. The customer is always right.

Well, NASA has managed to ignore engineer warnings that now led to the destruction of two (2) shuttles.
Perhaps three is the charm.
