I think I caught the Shepard Smith comment about the Indian astronaut. He mentioned that she had flown on another mission and she made some mistake in relation to a satellite and they had to send another astronaut out to retrieve it.

Sheppard Smith was listing the names of the seven astronauts on-board Columbia and making a brief mention of their NASA-related achievements. When he came to Kalpana Chawla, he mentioned that on her last shuttle flight (in 1997, I believe) she did something that caused a satelite to tumble briefly out of orbit.
