I remember from the cold war days that a nuke on a medium sized city would fill every hospital bed in the country with casualties.

Depends on the size of the nuke.

The ones Pakistan has are in the WWII range, much smaller than needed to do the damage you describe (but still very bad news).

The suitcase nukes the Russians reportedly lost are much smaller than the Paki version. They would supposedly destroy everything in a three block radius (with collateral damage out much further).
