I don’t think (could be wrong though) that the issue here is a technical breach or external hack—it’s a human failure in access control. Someone with access to the group added Jeffrey Goldberg, so the system itself didn’t fail—the protocol for managing who gets access did.

This is less about “IT security” in the traditional sense (encryption, network breaches, etc.) and more a procedural or operational security failure—someone inside the circle either made a mistake or acted intentionally.
