More likely that was some kind of sore. It shows up in one high school photo but not in other pictures, including the more recent problems. Kids with cleft palate or cleft lip do get grief from other kids but there’s no reason to add to those troubles by speculating about a connection between cleft lip or palate and mental illness.
I was curious about that as well. The flat nose combined with the middle part of the lip.
