I do suspect this accident is similar to other munitions-related accidents where pretty much all safety protocols are tossed aside in favor of increased production levels as it’s much cheaper to do that, rather than properly expand the facilities.

...and the experts who used to build them were let go or retired out a few years back and were replaced with the C Team.
