Looked like several engines failed to ignite. And, when the engines gimbled and flipped the vehicle over for stage separation, they got stuck and the rocket pinwheeled.

Looked like several engines failed to ignite.

I noticed that as well, and one of those that was not lit was one of the gimbaled engines.

Have to wonder if that affected maneuverability.
