Could be any number of mental issues. You might look up Body Integrity Identity Disorder or Xenomelia/apotemnophilia, both mental illness’ in which the patient desires the amputation of a limb or body part. Requires psychiatric treatment but good luck finding a surgeon who will “cure” the patient by amputation. Other than after botched attempts at self amputation. There’s a similar disorder relating to a wish to be blind. These are rare, often arise in childhood, and not treated by encouraging amputation, even in adults much less children.
