In criminal cases the right is absolute. In a civil case instituted by the government you can still plead the fifth amendment (because you could theoretically confess to a crime while the subject of a civil trial) but the judge can instruct the jury to infer that you have something to hide from doing so. That is impermissible in a criminal case.
