If NYC gets hit with an atomic bomb, millions, literally, will die instantly. Additional millions, literally, will die over the next few weeks due to radiation poisoning.
It won’t be pretty. Assuming the Empire State Building is ground zero and the bomb is on target, the entire Island of Manhattan will be unlivable for years.
However, the bombers will probably resort to an electromagnetic pulse or EMP rather than an atomic weapon. More people will live but will slowly starve to death. Drinkable water will be scarce. Electricity will be unavailable. In a way, the EMP will probably cause more misery than an atomic weapon.
